Define, at a high level, parts of the change request that are necessary for adequate assessment, including:

  • Business rationale
  • Possible impacts
  • Proposed deployment approach
  • Proposed remediation approach

Provide all information that will help the assessment team make a well-reasoned assessment.  It should be noted that simpler changes may have little information about these items.  Add this information to the change request form.
